Community Outreach:
We host outreach events every 4th Sunday of the month. This is a wonderful opportunity to connect with the community and provide essential support to those in need. Please review the below Volunteer Agreement and Release form. Once reviewed, please complete the volunteer form, and a member of our team will contact you within 24 hours. Please note all parties must complete the below form. Please be aware the minimum age to volunteer for on-site community outreach is 16 years of age.
Volunteer Agreement & Release Form
This Volunteer Agreement is intended to comply with applicable federal law and the laws of the State of Texas. This document includes food safety acknowledgments consistent with Texas Department of State Health Services (DSHS) and local health department expectations for charitable food distribution.
Volunteer Status & Texas Non-Employee Acknowledgment
I acknowledge that I am volunteering my services freely and without expectation of compensation. Under Texas law, I am not an employee, independent contractor, or agent of Adonai’s Redeemed Ministries and am not entitled to wages, benefits, workers’ compensation, or unemployment benefits.
Food Distribution Duties & Safety Compliance
As a Food Distribution Volunteer, I agree to follow all food handling, sanitation, and safety procedures provided by Adonai’s Redeemed Ministries, including those required by the Texas Department of State Health Services (DSHS) and applicable local health authorities.
Texas Health Department Food Safety Acknowledgment
I acknowledge and agree that:
• I will wash or sanitize my hands before and during food handling activities
• I will wear gloves or other protective equipment as instructed
• I will not handle food if I am experiencing symptoms of illness (including fever, vomiting, diarrhea, or contagious conditions)
• I will follow temperature control guidelines for hot and cold foods
• I will prevent cross-contamination between raw and ready-to-eat foods
• I will follow all instructions related to food storage, labeling, and distribution
I understand that these practices are consistent with Texas DSHS Food Establishment Rules and local health department guidance for charitable food service operations.
Code of Conduct
I agree to conduct myself in a respectful, ethical, and Christ-centered manner, to follow ARM leadership direction, and to comply with all applicable laws and safety requirements.
Confidentiality Agreement
I agree to maintain the confidentiality of any personal or sensitive information related to individuals served by Adonai’s Redeemed Ministries.
Texas Liability Release & Assumption of Risk
I understand that participation in food distribution activities involves inherent risks, including lifting injuries, slips and falls, and potential exposure to foodborne illness. I voluntarily assume all such risks.
To the fullest extent permitted by Texas law, I release and hold harmless Adonai’s Redeemed Ministries, its directors, officers, employees, and volunteers from any claims arising from my participation, except in cases of gross negligence or willful misconduct.
Medical Treatment Authorization
I authorize Adonai’s Redeemed Ministries to seek emergency medical treatment on my behalf if necessary.
Background Check Consent (If Applicable)
I consent to a background check if required for my volunteer role.
Photo & Media Release
I grant permission for Adonai’s Redeemed Ministries to use photographs or recordings of me for ministry and promotional purposes.
Minors
All volunteers who are minors must be at least 16 years of age. Minors are required to be accompanied and supervised by their parent or legal guardian at all times during volunteer activities. Due to safety reason, a minor under the age of 16 years of age are not permitted.
By allowing my child to participate, I acknowledge and agree that, to the fullest extent permitted by Texas law, I also release and hold harmless Adonai’s Redeemed Ministries its directors, officers, employees, and volunteers from any claims or liabilities arising from my child’s participation, except in cases of gross negligence or willful misconduct.
Governing Law
This Agreement shall be governed by the laws of the State of Texas.
